132. A petition shall be signed and dated by the petitioner and shall specify—

(a) the European election to which it relates,

(b) the grounds on which it is based,

(c) the remedy it seeks,

(d) the name and address of the petitioner and of the petitioner ’ s solicitor or agent, if any.
